INSS: add healthcare beneficiaries

Check the route for adding eligible family members to a recognised healthcare entitlement.

Sources checked · 03.11.2026

Choose the right procedure

The Alta de beneficiarios service lets a titular request inclusion of a person without their own recognised entitlement, subject to the applicable conditions. A family relationship alone does not guarantee inclusion. Check both people’s existing entitlement before choosing this route.

What to prepare

Prepare the titular’s and proposed beneficiary’s identity details, and the family or dependency evidence requested by INSS. Check the current requirements for foreign civil-status documents. If acting for someone else, follow the service’s representation or power-of-attorney requirements.

Spanish fields explained

Titular identifies the person whose entitlement is used; beneficiario identifies the person to be added. Parentesco is the relationship. A representative is not automatically a beneficiary. Adding a family member to healthcare does not grant that person immigration residence.

How to submit

Open the official Alta de beneficiarios service and follow the identification options offered. Adding a beneficiary requires stronger identification than simply checking entitlement. If you also need recognition as a titular, the INSS recognition application can include beneficiaries in the same form.

After submission

Save the document produced by the service. Depending on the case it may be proof of entitlement or only a submission receipt pending INSS checks. Read its heading before treating it as approval. After recognition, follow the regional health service’s instructions for the beneficiary’s health card.
